Mother Lota in Vilna. Mother Khaya deceased.

Signed Avraham Tservin (in Hebrew). Wife's mark.


On the Hebrew side:

"Berkovna" becomes "bat Dov" (Dov=Ber).

"Street" is abbreviated by resh with a bar over it.

There seems to be only one witness, Shmuel Segal.
