• Left: date
• Upper middle, descending: Kovno [Mo]dern
• Upper right: Zum Itz[hak ...] for ...
• Lower right: Fon mir ayer aynikel Yekv Tshervin

Translations, right:

• Upper: "To Isaac ..." (assuming that Itzhak begins with a "shtumer aleph" in Yiddish, which needs confirmation).
• Lower: "From me your grandson Jacob Chervin
